Settlement procedure through the Insurance House

USEFUL INFORMATION

Medical Express products can be obtained FREE of charge or PARTIALLY COMPENSATED, through the County Health Insurance House in the area where you reside, based on the decision approved and issued by this institution.

Important! The documents listed below are for general guidance only. Because each County Insurance House may request more or fewer documents, please contact us at 031 433 6111 before submitting your file .

Documents required to obtain the decision of the County Health Insurance House :

  • Type request;
  • Supporting document attesting to the insured person's status (pension certificate, employee certificate, unemployment benefit receipt, etc.);
  • Copy: identity card / identity card;
  • Medical recommendation (prescription) in 2 copies from the specialist doctor with the mention “modified set” and the patient’s signature – for more details about its necessity, call 031 433 6111. 1 copy is required for the Health Insurance House and 1 copy for the doctor. (The recommendation must specify the Health Insurance House with which the medical unit is in a contractual relationship and the contract number); If you wish to receive a model of the medical recommendation (Annex 39B), do not hesitate to contact us;
  • A self-declaration stating that the organic or functional deficiency did not occur as a result of occupational diseases, a work or sports accident;
  • If you have children up to 14 years old: copy of birth certificate, school certificate (if applicable).

If you have no income , the patient becomes a co-insured of a family member (degree 1). In this situation, the file must contain:

  • Copy of patient's (co-insured) ID/ID
  • Copy of insured person's ID card (husband, wife)
  • Copy of marriage certificate
  • Proof of insurance status (pension certificate / employee certificate)
  • Tax certificate (proving that the patient has no income) from the city hall – in the country or from the financial district of the sector where the patient lives
  • Medical recommendation (in original)

For children under 18 years of age , the following documents are required:

  • Copy of child's ID / ID card
  • School certificate (primary/middle school/high school/vocational school)
  • Copy of the BI / CI of one of the parents
  • Proof of the insured capacity of the respective parent
  • Medical recommendation (in original)
